This new three-day course meets the knowledge
requirements of two of the LDAF Mandatory Units at Level 3.
It focuses on all aspects of the assessment and care
planning process; its objectives and principles, the
legislative framework, the tools used in the assessment
process and the importance of a positive, person-centred,
holistic approach. It stresses the importance of service
user & carer involvement, observation, consultation,
multi-agency working and teamwork throughout the cycle of
planning, implementing, monitoring and evaluating care
plans. The course is open to candidates from the
Partnership Trust, the three PCTs, Gloucestershire Social
Services, private and voluntary services. Course fees for
private and voluntary services are detailed at the back of
this directory. Meets knowledge requirements of
LDAF Level 2 Optional Units:
- 012 - Contribute to the assessment and care planning
process (Double Unit)
- 013 - Contribute to the monitoring, evaluation and
review of care plans
- 015 - Understand advocacy
Meets knowledge requirements of LDAF Level 3 Mandatory
Units:
- 101 - Undertake assessment and care planning
- 102 - Monitor, evaluate and review care plans
Shares knowledge requirements with NVQ Units &
Occupational Standards:
