How to apply for study leave
Applicants from Glos. PCTs and Partnership NHS Trust
Discuss your application with your manager to agree
relevance of study leave in line with service/departmental
objectives and as part of your Personal Development Plan.
Telephone Collingwood House to book your place on the
course, unless otherwise stated on course page.
If there is a place available:
- Complete a study leave form
- Manager and applicant sign study leave form
- Form retained by manager - please do NOT send
completed forms to Collingwood House
- Budget holder signs to agree study leave, and
percentage of fees and expenses where applicable
- All correspondence relating to places on in-house
courses will be sent to the individual concerned
approximately two weeks before the course date.
- If you are unable to attend a study day or course
after you have booked on, please let us know as soon as
possible so that your place can be reallocated.
Study leave forms (DP1) can be ordered from the Buying
Team at GRH - Product Reference No. WKX860T. Please do NOT
send completed study leave forms to Collingwood House.

Directory for external applicants. For information about

Limited places are available for applicants NOT employed
by the Partnership Trust or the three Gloucestershire PCTs.

- One-day Course: £ 55.00
- Two-day Course: £ 90.00
- Three-day Course L/D: £120.00*
- Foundation Course Working with People with Learning

- Handling & Moving for Keyworkers: £250.00

Cancellation fees are applicable as given below:
Cancellation between:
- 4 to 6 weeks before the date of the event - 20% of
total booking value
- 2 to 4 weeks before the date of the event - 50% of
total booking value
- Less than 2 weeks before the date of the event - 100%
of total booking value
